You are correct in your subcategory tag above. You have some corruption in your database. How much is hard to tell from your post, and is always something to look at carefully when it happens during an approval as there are several steps to approving a bill and we don't yet know which ones got done before the hiccup. Unless you have a backup that you could restore and try again, you will need to have the database repaired.
