Do you have a long scroll bar at the bottom of the screen when you try to add a new transaction? You may have inadvertently dragged the entry window so far off the screen that it is now out of sight. As Caren has mentioned, going to Setup | Preferences | Interface/Appearance and clicking the "Restore all dialog settings to their defaults" button should bring it back if that is the case. (Make sure all dialogs are closed when you do this. The easiest way to do this is to just exit the program, then come back in and try the fix before you do anything else.) The other errors are a different problem. It relates to the Client Custom Date field named Closed on three of your clients. We have seen some clients with issues related to custom date fields in 2015. If you know which records, you can go in and fix them, but that would not be causing the inability to enter a transaction.
